Statement of Purpose: 
Indiana Care Circle (ICC) exists to enhance continuity of care as an essential component of the health care delivery system and an integral part of the total patient care.

ICC accepts the responsibility of promoting the development and systematic implementation of the highest standards of continuity of care.

ICC serves as a medium for the sharing of professional problems, solutions, and resources in the development of health care programs.

ICC also serves to promote administrative awareness of the standards, structure, and organization of a continuity of care program.

Continuity of Care

The World Health Organization defines Continuum of Care as:  
"the provision of comprehensive care from the hospital to the home, which advocates the pooling together of medical and social services within the community and the creation of linkages between community care initiatives at all levels of the health care system."

Indiana Care Circle is dedicated to continual improvement of working out this concept in the Tippecanoe County area.

Field of Interest: Elder Care 

Amount available: $716

To enhance continuity of care as an essential component of the health care delivery system in Tippecanoe County, Indiana

The Community Foundation of Greater Lafayette

During our April, 2021, meeting, Candy Silver, Director of PhilanthropyCommunity Foundation of Greater Lafayette talked with us about the ways this local 501 (c)3 organization benefits our community.

The Community Foundation of Greater Lafayette oversees more than 250 funds and trusts, distributing over 1.8 million dollars in local grants and scholarships (in 2020).

Candy explained the various funds, which includes ICC's own "Field of Interest Fund."

ICC makes an annual contribution to our fund, but ANYONE can contribute any amount at any time. This includes providing for the fund in your will by leaving a "legacy gift."  

Contact the Community Foundation to contribute to our fund. Ask about the Indiana Care Circle Endowment.

We began making annual contributions to the fund in 2016. In 2020, our endowment awarded Tippecanoe County Council on Aging $547 for supplies for the center.
